 3. Return here to complete the Questionnaire.

​Please complete our Questionnaire, the Questionnaire provides me with a comprehensive understanding of your Health and Wellbeing.

When you make your booking:

  • If you select Telehealth, your appointment will be conducted via your computer or phone from the comfort of your home. 

  • If you select a Face to Face Consultation, I will see you in my rooms on the day of your appointment. I am fully vaccinated.
